SPECIAL NOTE: Please take a few minutes to read through this. Of particular note are the following:

*   Yes, please send me your completed form/questionnaire even if you participated last year.  I do update the form every year, so there are minor changes, but also it is a hassle for me to hunt around for last year's documents and emails.  If you are pressed for time and you still have it, you can re-send me last year's completed form but please make sure that the required fields and other information are still current.

*   By participating in this gift exchange, you are agreeing to send your package to arrive no later than the target date of Friday, December 23 (there may not be any Saturday delivery service). Factors that you should take into consideration ahead of time include 1) distance/location (Within Same State/Coast to Coast/Canada); 2) shipping service used (Postal Service/FedEx/UPS); and 3) shipping rate used (Priority/Parcel Post/Ground). Please see Section 4 for more details on dates and deadlines.

*   International Shipping: Please note that some of the participants reside in Canada, and shipping internationally (US to Canada/Canada to US) generally involves a higher shipping rate ($28-$36 for a decent sized package) and requires additional time to reach its destination. If either poses a problem, please let me know at the time of sign up. Please see Sections 7 and 11 for additional information on shipping time and shipping rates.
